These people are considered unwanted by psychiatric structures – both public and private – and social services.

They are the cases that are referred from one service to another, even when they have the character of urgency, even after a suicide attempt or other acts of despair. Psychiatric hospitals "do not accept drug addicts." Therapeutic communities, as a rule, with some exceptions, "do not accept psychiatric cases." Ultimately, no one knows where all these people belong.

For those who seem to belong nowhere, those who cannot find a place to stand, the outcasts of society, the psychiatric system, and detoxification programs, this book speaks. For those who everyone considers "difficult patients," "without a treatment request," "with a poor prognosis," both in terms of mental disorders and in terms of detoxification.

For those, our clinical experience proves that the therapeutic relationship within the appropriate therapeutic framework, where a "constellation of groups" operates, in the words of Félix Guattari, with the main tool being psychotherapy and Art in dialectical relationship, can shape all the terms of treatment, that is, their transformation into social subjects, subjects of emancipation and subjects of creation, free from dependencies and without psychopathological manifestations.

The book presents some clinical cases, representative of the psychiatric nosological entities we encounter most often in this field (psychoses, emotional disorders, pathological personalities).
