At certain points, emphasis has been given, while on the contrary, material deemed misleading has been omitted. The removal of references from the text in the first 14 chapters and their transfer, with some economy, to the end of each chapter represents a change in the presentation of the text.

These and other changes aim to make the text even more accessible and relevant to a plethora of different clinical specialties. At the same time, we maintain documentation for those readers who seek a more in-depth study and research.
