The presentation of the game includes:
- Role of the Conductor: One of the players assumes the role of the Conductor and must choose which railway track to send the crime down, hitting those in its path.
- Teams: The other players are split into two teams and try to convince the Conductor to save those on their track, killing those on the opposite one.
- Strategy: Each round, one player is the Conductor, while the others place heroes and Modifier cards on the tracks, trying to influence the Conductor's decision.
The board with the railway tracks is placed in the center of the table, and the game cards are separated into:
- Innocents: Positive characters who have done something good or will have a positive impact on the world.
- Guilty: Negative characters who have done bad things or will bring negative effects.
- Modifiers: Cards that change the perception of the Innocents and the Guilty.
The players draw cards and place them on the railway tracks, influencing the perception of the "condemned" and the decision of the Conductor. The railway track that is spared wins the round. The "slaughter" in Trial by Trolley continues until all players have taken on the role of the Conductor and felt the weight of the choice.
